Prep instructions and forms for your procedure.

Everything you need before your colonoscopy or endoscopy at Hamptons Endoscopy Center — bowel preparation instructions, new patient forms, and what to expect on the day of your visit.

Procedure instructions

Bowel preparation instructions.

Preparations are used to cleanse the colon so the lining can be properly visualized, allowing your physician to identify polyps, growths, and blood vessels. Following the instructions properly can be essential to a successful procedure — and helps prevent needing a repeat procedure at a sooner interval.

Use the prep your physician prescribed — and match it to your appointment time

Based on your procedure time, you will have been given either a split-dose or a same-day preparation. Select the link for your prescribed prep below and follow the instructions for before 9am or after 9am appointments, matching your scheduled time.

Prep instructions are specific to each physician and each patient. If you are unsure which prep you were prescribed or which version to follow, contact your physician's office before you begin.

Forms

Patient forms.

Please print and complete these forms so we can expedite your first visit. You will also need to complete them if your last visit was more than a year ago.

Procedure instructions

General instructions — not bowel prep instructions

Colonoscopy instructions cover the day of your procedure. For bowel preparation, use the prep instructions above.

Forms are provided as PDFs. You'll need a PDF reader such as Adobe Acrobat Reader to view or print them.

What to expect

Being well-prepared makes for a better visit.

Arriving well-prepared ensures your physician has all the information needed to provide the best possible care — and helps ease any unnecessary anxiety before your appointment.

Take some time to read about your symptoms in our services section, and to review our physicians so you're familiar with the specialist you'll be seeing. We look forward to your visit.

Bring these to your appointment: ID, insurance card, and your completed forms.

Also bring a current list of your medications, including dosages. Because your procedure involves sedation, arrange for a responsible adult to drive you home and plan to rest for the remainder of the day.
